Tried Ogre Lord Isle for the first time. Got swarmed and killed. Mule died too. That's when the fun started. I started to run for a res when I realized my mule wasn't following me. I looked all over and couldn't find her. I could see Sammy's name flicker but couldn't find her. It finally got to the point where I'd go to sleep and try today. I was still ghosting when I found Sammy'd gone over the side of the isle and couldn't come back up. I also could not see Sammy to target her.
I spoke over global to have Magneto and another to show up to help me. They killed all the OLs and ressed me. Still couldn't help Sammy and discovered something else. All my insured items and even my newbie stuff (pants) were gone. Just my blessed books, reward cloak, and trash bag remained.
With 26k+ in the bank, gold, they all should have been covered, but the most important item was a two-handed Japanese sword, level 100, called Darkened(or ing) Skies.
What followed was me streaking in my cape from bank to home trying to find where they might be. No luck.
Sammy's still stuck, and I'm left with the feeling that if this kind of night had been in real life, I'd probably be in jail or the hospital. lol.

You can use a get-followers stone (IIRC there are ones west of britain bank and near the taming forest world teleporter) to fetch sammy's bonded ghost. If you still have access to the newbie dungeon, the one there is free. Make sure you have insurance set to renew (click yourself and select "renewing inventory insurance"). Otherwise it only works once. Just one small tip, never go offline after you died. It feels weird, no matter how tired you are 
I once lost all my pets when I was too tired just because I logged out without shrinking them...
I hope you find some new gear soon. It's sad to see all of it is gone
And like said before, put your insurances so that they are always renewed.

For future reference, there's an ankh for a free res at the entrance of every dungeon. Despise would be one of the easier ones to get to and back again before your corpse decayed. With the use of cleric spells (even if they're eating into your piety), hiding, and unlimited stealth, you should be able to get back to your corpse easily almost anywhere. I've made it from Felucca Hythloth level 4 to the entrance and back again; it doesn't get much harder than that. Sorry to hear about your lost items; that always sours a good day's adventuring.
