Packet Loss Issues w/Traceroute Pic, Need Help

I've been experiencing 100% packet loss from 2 out of 16 hops to the game server for several weeks now. I've been waiting to see if the problem would be resolved but it hasn't been. So I have choppy connection, random disconnects, you know. The 2 hops that are hurting me are the ones at the very end, right before the game server itself. I have no idea what to do to fix this. It takes an act of God to get my own ISP to resolve technical problems, so I don't hold out any hope for a strange ISP listening to my tale of woe. I'd very much like to get this fixed somehow and improve my game experience here -- if anyone has any idea of how I can achieve those goals, please share. Thank you
